Over 60
Under 16
Under 19 in full time education
Over 40 with a ﬁrst degree relative with glaucoma
Diagnosed as having diabetes or glaucoma
Advised by an ophthalmologist that you are at greater risk of glaucoma
Registered partially sighted or blind
Have a complex prescription

You will receive NHS help towards the cost of your glasses if you or your partner is receiving:

Income support or income based jobseekers allowance
Working tax credit
Child tax credits
Named on a certiﬁcate for full help with health costs, an HC2
Named on a certiﬁcate for partial help with health costs, an HC3, you may get some help or have been prescribed complex lenses
